Everyone needs to sign this form and submit it to your Couple Coordinators.
UNDER 18: PARENTS sign it.
OVER 18: YOU sign it.

REGISTRATION IS CLOSED

No worries though! If you have other member who want to come but didn't get a chance to register, there will be registration forms ready at the event. They just won't get fancy name tags.  ALSO....don't forget to print out the waiver form.
